If you are requesting an appointment, please let reception know immediately if you require an urgent or non-urgent appointment. Urgent consultations will be transferred to a nurse or doctor to ascertain the seriousness of your problem and we will accommodate you as soon as possible or will call for an ambulance.

Attending your Appointment We are located in the Botany Town Centre, level one. Enter the sliding doors on the east side of the upper car park and turn right to go down the corridor by Horizon Radiology.
On arrival, please let reception know that you have arrived for your appointment. Appointments are 15 minutes and an extra charge may apply for extended consultations.
After Hour Care and Emergencies We are not open on weekends or statutory holidays, however we provide an AFTER HOUR TRIAGE NURSE SERVICE – call 09 273 6003 and you will be redirected to someone that can help or assist.
For life-threatening injuries or suspected heart attack, call 111 or for Urgent Medical Care, please call (09) 277 1516 or visit East Care A&E at 260 Botany Road.
They will advise our practitioners of any visits after hours.

Registration with the PHO If you are a new patient to the practice, or visiting as a Casual, you will be asked by our reception staff if you wish to register with the practice.
We are currently accepting new patients and invite you to call in and complete a registration form or download a copy of our enrolment form and bring a signed copy with you to complete the registration process. You can download our Information Form here and our Enrolment Form here.
Please note: In order to enrol at any general practice in New Zealand, the Ministry of Health require us to sight specific documents to confirm eligibility for funded health services. Therefore, along with your completed enrolment form we require:
1) A photocopy of your passport with any relevant permits/visas to verify eligibility or your NZ birth certificate.
2) Two further forms of identification, one being a photo ID (unless a passport was supplied).
These are a requirement for New Zealand citizens also.

Benefits of enrolment: Registering enables you to apply for enrolment with us as part of our PHO, East Health Trust, and enjoy the subsidised fees due to additional funding from the Ministry of Health.
Reduced consultation fees for registered patients from the time that the Ministry of Health enrolment and funding has approved.
Cheaper prescription charges.
Assistance with obtaining government funded mammogram Screening, diabetic education and dietary advice, access to podiatry services and diabetic eye screening.
Primary Options – subsidised care for patients treated at the centre that would otherwise be admitted to hospital.
Cervical Smear Project – FREE cervical smears for high-needs patients, certain criteria apply.
All our registered patients, who have consented, will benefit from our recall system. When you have a procedure done or are due to be checked, we will send you a reminder letter.

o Cervical Smears o Mammograms o Immunisations and vaccinations o Depo Provera Contraception Injection o Routine Health Checks & screening including cholesterol, heart disease, diabetes, etc. o IUCD
